What is melasma?

Melasma is chronic and easily provoked, so the modern approach is gentle, layered and consistent rather than aggressive. We first identify the type — epidermal (surface), dermal (deeper) or mixed — and the triggers, because depth and drivers determine what is safe and effective for your skin.

A typical plan layers daily sun protection (the foundation), doctor-guided topicals that calm pigment production, carefully-dosed low-energy laser and barrier-supporting care. The goal is meaningful, lasting control — keeping melasma faint and stable — rather than a one-time "cure" that risks rebound darkening.

Five parts of a melasma-control plan

1. UV and visible-light protection

Use broad-spectrum SPF 50+ consistently, reapply when exposed, seek shade and add hats or umbrellas. A tinted sunscreen containing iron oxides may help selected patients exposed to visible light. Photoprotection is the foundation for reducing relapse; no device replaces it.

2. Prescribed and brightening topicals

A doctor may select prescription pigment-suppressing medicines and supportive brightening actives according to skin sensitivity, pregnancy status and prior irritation. More products are not always better: excessive layering can inflame the skin and worsen pigment.

3. MesoBright as an adjunct

Hori Clinic may add MesoBright to selected plans after reviewing the formula, allergies, pregnancy and medicines. It is an adjunct for skin-quality and pigment care, not a replacement for diagnosis, photoprotection or prescribed topical treatment.

4. Low-energy Pico Majesty, often planned for 6–9 sessions

For suitable melasma, the clinic may plan a gentle low-energy Pico Majesty series of about 6–9 sessions rather than one aggressive treatment. The actual number, interval and wavelength depend on melasma depth, skin type, response and PIH risk; published picosecond evidence is mixed across wavelengths.

5. RedTouch Pro 675 nm: three monthly sessions in a Thai study

A prospective pre–post Siriraj study in 28 Thai women with Fitzpatrick III–V reported a mean 41% mMASI reduction at 3 months after three monthly 675 nm sessions, and 34% at 6 months. This small single-arm study had no comparison group, so the result is promising but does not guarantee an individual outcome.
